Integrated community supports (ICS)

Services that provide support and training in community living service categories to adults age 18 and older who reside in a living unit of a provider-controlled ICS setting (e.g., apartment in a multi-family housing building). ICS can be delivered up to 24 hours per day in the person’s living unit or in the community.

24Hr Customized Living

An individualized package of regularly scheduled, health-related and supportive services provided to a person 18 years or older who resides in a qualified, registered housing-with-services establishment 24/7.

Medication Management

Our staff prioritizes and manages all our client’s need for psychotropic medications, the provision of a prescription, and ongoing medical monitoring related to the patient’s use of the medication.

Meal Preparation

We provide three healthy home-made meals per day, and snacks. We use fresh ingredients and cater to our residents preferences.

Rehabilitation

Rehabilitation is care that can help you get back, keep, or improve abilities that you need for daily life. These abilities may be physical, mental, and/or cognitive (thinking and learning).

Transportation

All transportation needs are provided to appointments, frequent social events, or personal errands.

Housekeeping

Our Qualified Caregivers have the experience and compassion to provide assistance with everyday personal care tasks.

Personal Care

When our residents need assistance with bathing, dressing or any level or personal care, we are there to provide it.

Support Services

Activities include walks, movies, bowling, shopping, music therapy, cognitive therapy, crafts, gardening, birthday and holiday celebrations and more.
